ICI vs. IUI at home: the honest comparison
Two acronyms get used interchangeably online, but they are not the same procedure. Only one of them is safe to do at home. Here is the difference between intracervical insemination (ICI) and intrauterine insemination (IUI), and how to choose.
ICI — Intracervical insemination
A soft, needleless syringe places the sample at the opening of the cervix. The sperm then swim through the cervix and into the uterus the same way they would after intercourse. ICI can be done safely at home with a sterile syringe — no medical training is required.
IUI — Intrauterine insemination
A thin catheter is threaded through the cervix and deposits washed sperm directly into the uterus. This is a clinical procedure. Sperm must be lab-washed because raw semen in the uterus can cause severe cramping and infection. The catheter insertion also requires training.

Why ICI is the safer at-home option
Putting unwashed semen into the uterus is genuinely dangerous. Raw semen contains proteins and bacteria that can trigger severe cramping and infection when placed inside the uterus. Sperm washing requires a centrifuge and trained technician. ICI avoids that risk by placing the sample where the body is built to receive it: at the cervix.
If you have tried 6 or more well-timed ICI cycles without pregnancy and you have access to a clinic, IUI is a reasonable next step. Until then, ICI at home with a fresh or shipped sample is statistically close and far less invasive.
What you need for ICI at home
- A sterile, single-use needleless syringe with a rounded tip
- A sterile collection cup
- Accurate ovulation timing (LH strips, BBT, or cervical mucus tracking)
- A tested known donor or a sperm bank ICI-ready vial
- If shipping: a semen extender and insulated overnight packaging
